In short: SmplForms texts you only when an organization you deal with (your doctor's office, school, law firm or another business) has your mobile number and your agreement to be texted about its forms. Reply STOP to stop, HELP for help. Message and data rates may apply. Message frequency varies.
1. Who sends these messages
SmplForms is a form and automation platform operated by Durga-Dagda Technologies LLC. Organizations that use SmplForms ("Tenant Organizations") send their forms and notices through it. Text messages come from the SmplForms toll-free number +1 (833) 593-3904 and name the organization they are sent for.
2. What the messages are
Messages are transactional only. They include:
- an invitation to complete a form, with a secure link to it;
- reminders about a form you have not finished;
- notices that a document is ready or that an appointment slot is booked;
- one-time sign-in codes when you choose to receive them by text.
We do not send marketing, promotional or political messages, and Tenant Organizations may not use SmplForms to send them.
Example: "Acme Health: please complete your intake form before your visit: https://acme.smplforms.com/s/… Reply STOP to opt out."
3. How you agree to receive them
You receive texts only after you give your mobile number to a Tenant Organization and agree to receive text messages from it about its forms and services, for example on its intake or registration form, in its patient or client portal, or by telling its staff. The Tenant Organization records that agreement and enters your number in SmplForms; SmplForms will not text a number the organization has not added. Agreeing to texts is never a condition of receiving care, service or purchase. You can ask the organization to use email instead at any time.
4. How often
Message frequency varies. Most people receive a few messages per form they are asked to complete, and none when they have nothing outstanding.
5. How to stop
Reply STOP (or CANCEL, END, QUIT, UNSUBSCRIBE) to any message. You will receive one confirmation and no further texts from that number. Reply START to opt back in.
6. How to get help
Reply HELP to any message, or email support@smplforms.com. For questions about a specific form, contact the organization that sent it.
7. Costs and carriers
Message and data rates may apply, according to your mobile plan. Carriers are not liable for delayed or undelivered messages.
8. Your privacy
Mobile numbers and text-message consent are never sold, rented or shared with third parties or affiliates for marketing or promotional purposes. Numbers are used only to send the messages described above. See our Privacy Policy.
